Root Cause Anaysis |
A technique that is used to determine the underlying root causes of risk, variance, or a defect.
Often Cause and Effect Diagram is used to find the root cause.
It is used in Risk Management to identify the main cause of anticipated threats (and opportunities).
It is used in Quality Management to find out the main cause of a defect. After the root cause is found and resolved the entire problem vanishes.
It is used in Stakeholder Management to identify and resolve the issues related to stakeholder expectations and support.
EXAMPLE
Let us say a project is over-budget. At the outset, it seems like there is something wrong with the estimate or incurred expenses. But it was discovered that a new hire is not skilled in the required technology and throws a lot of defects to the systems attracting reworks and thus extending the timeline. So the root cause of the over-budget can be an unskilled new joiner.
